GENERAL SUMMARY:

Salary: $22.62/hour

The SNAP Outreach Coordinator is a staff member of the Sister Visitor Center, a department of Catholic Charities of Louisville, Inc. located in West Louisville. The SNAP Outreach Coordinator is the primary point of contact for assisting Louisville community members in their SNAP enrollment, and is responsible for conducting outreach to connect people with SNAP benefits, including pre-screening for eligibility, application assistance and recertification, information dissemination to clients and staff of partner organizations, following up with clients, and data tracking/reporting. This position works closely with other partners to connect individuals with other information, services, and referrals. The SNAP Outreach Coordinator must be familiar with the Sister Visitor Center Choice Food Pantry operations, to make appropriate referrals to that service and to provide food pantry support/coverage as needed.

ESSENTIAL DUTIES AND TASKS:
  1. Develop an in-depth understanding of the SNAP (food stamps) enrollment process.
  2. Work with community partners and CCL programs to assist participants in enrolling in SNAP.
  3. Assist participants with gathering and uploading required documents for eligibility.
  4. Follow up with participants to assist with the re-certification process to prevent loss of SNAP benefits.
  5. Maintain up to date case files on participants. Assist with copies of scanned documents to aid re-certification and troubleshooting cases that are not approved.
  6. Accurately record information in agency and other related databases as needed to ensure accurate monthly or yearly reporting, and to comply with funding requirements.
  7. Contact SNAP offices and stakeholders to follow up on case information and outcomes.
  8. Provide referrals to other Coordinators, other CCL service areas, or other community services.
  9. Conduct outreach to eligible residents of Jefferson County.
  10. Complete program reports to the supervisor and funding sources.
  11. Make appropriate client referrals to the Sister Visitor Center Choice Food Pantry and other food resources, as needed.
  12. Assist the Sister Visitor Center team in the Choice Food Pantry during high service demand or staff shortages, as needed.
  13. Other duties as assigned by supervisor.

EDUCATION AND EXPERIENCE:
Bachelor’s degree or at least 2 years of social services experience is preferred.

• Must have knowledge of and familiarity with community resources.
• Must have valid driver’s license and personal transportation.
• Must be proficient in PC skills including Microsoft Office applications such as Word, Excel, and Outlook
• Experience and willingness to learn new technologies including Kynect software and client track
