Linkage Navigator - Full Benefits Package
Full-time, 37.5 hours per week; Grade 7, $24.80 per hour with a full comprehensive benefits package.

The primary goal of the Linkage Navigator is to link Human Immunodeficiency Virus (HIV) and/or Acquired Immune Deficiency Syndrome (AIDS) positive individuals who have not received care in the previous six months back into medical care. In addition, the incumbent will guide these individuals through the re-entry to medical care process. The incumbent will help identify out-of-care HIV positive individuals in the Care Coordinator Program and be responsible for reinitiating contact with them and linking them to eligible services. The incumbent will assist in getting newly diagnosed HIV positive individuals into treatment, when necessary. The Linkage Navigator will assess the psychosocial needs of each client and link the client to other appropriate health care and social services, as well to the client’s HIV provider and care coordinator. This position will act as a liaison to HIV primary care, medical and other core and support services. In addition, the incumbent will provide disease investigation, partner services and linkage to care for individuals newly diagnosed with HIV and/or syphilis.
This position requires daily travel to and from job assignments.
Bachelor's degree (BSW or other closely related health or social work field) or equivalent from an accredited college or university with one (1) year of case management experience or (1) year of experience in HIV field with patient contact; OR an Associate's degree plus four (4) years of case management experience or four (4) years of experience in HIV field with patient contact; OR HS/GED plus seven (7) years of case management experience or seven (7) years of experience in HIV field with patient contact. An advanced degree in Social Work (Certified MSW) or Mental Health Counseling may substitute for the 1 year of related experience.
Relevant experience includes professional work in social work, patient/client interviewing, community health, professional counseling, mental health services or health education.
Training in blood borne pathogens, phlebotomy, HIV pre-test and post-test counseling (within 6 months of appointment), and the Passport to Partner Services Course Required within one year of appointment.
