Now hiring for a full-time Diabetes Population Health Specialist, 37.5 hours per week, $28.30 per hour with a full benefits package. 

The Diabetes Care and Education Specialist provides community-based diabetes education services to persons in community settings for the purpose of diabetes prevention, management and support.  Provides evidence-based public health strategies for population-focused diabetes prevention and control activities in alignment with the Department for Public Health’s Kentucky Diabetes Prevention and Control Program (KDPCP). 

Must have one of the following education/experience combinations: Applicable Bachelor’s Degree with 2 years related experience or Master’s Degree with 1 year of related experience. 

Acceptable related experience includes some or all of the following in diabetes specific populations: demonstrated group facilitation skills providing diabetes education and support to groups; community networking and outreach.   

Current licensure required by official credentialing agency is required if a licensed health care professional. Current CPR.  A Certified Diabetes Care and Education Specialist (CDCES) or Board Certified Advanced Diabetes Management (BC-ADM), or willing to become credentialed within 9 months of hire. A Licensed Diabetes Educator (LDE) or Master Licensed Diabetes Educator (MLDE) or willing to become licensed within 9 months of hire date.
