The Lexington-Fayette County Health Department (LFCHD) is an independent public health agency charged with helping Lexington be well since 1904 and proudly nationally accredited since 2014. Vision: The Lexington-Fayette County Health Department is a recognized leader in health promotion, health protection, personal health care and prevention of disease. Mission: The Lexington-Fayette County Health Department will improve the health of the community. In 2014 LFCHD became nationally accredited by the Public Health Accreditation Board, continued quality improvement, vaccinated 216 children, provided nutritious foods, education and referrals, provided safe and sanitary establishments when dining out, enforced the Clean Indoor Act, protected 462 Lexington pets against rabies and protected 1,921 Lexingtonians against the flu. Additionally, LFCHD responded to more than 800 reports of disease and illnesses in Fayette County, provided 2,951 TB clinical service visits, provided 5,596 HANDS home visits to first-time moms and dads, promoted the evidenced-based diabetes prevention program, provided training to about 600 licensed child care providers, improved the nutrition of adults and children, helped individuals quit using tobacco, eliminated health inequities, remained attentive to the health of more than 40,000 public school students, and was awarded with a Bluegrass Wellness at Work Healthiest Places to Work Award. The Nurse Specialist provides nursing services focusing on the prevention of illness, injury or disability, the promotion of health, and maintenance of the health of individuals and populations. The incumbent works independently within agency guidelines in a variety of settings to provide services.
Specifically, the Nurse Specialist provides nursing services focused on identification, control, treatment and prevention of communicable diseases, administration of immunizations, assisting clinician with family planning patients, and evaluation and treatment of sexually transmitted infections; provides direct patient care to vulnerable populations and practice case management with these patients to effectively control tuberculosis infection or active disease in both clinical and home field based settings; works with a diverse multidisciplinary team to positively impact the health of the community with regards to communicable disease control and prevention.
Communicable Disease Nursing duties but not limited to:
Provides surveillance of tuberculosis infection and disease by reconciling pertinent medical documentation, executing standard order for radiology, collecting diagnostic specimens and isolating patients following the Kentucky Department of Public Health (KDPH) Tuberculosis (TB) Control & Prevention Program guidelines contained in the current KDPH Core Clinical Services Guide. Provides comprehensive case management by coordination and follow-up on plan of care for patients diagnosed with and suspect for TB disease or infection, their families, and other contacts.
Collaborates with health professionals, other agencies and multidisciplinary team members regarding referrals as appropriate.
Provides Directly Observed Therapy (DOT) and directly observed prophylactic therapy (DOPT) to TB patients in the field or in a clinic setting per clinician’s order by coordinating a time and place that the patient and nurse agree upon. The nurse will observe the patient taking their medication. The nurse assesses patients for signs of drug toxicity by evaluating them for any symptoms. Makes a decision regarding whether to administer medication, draw blood for any indicated labs and perform a vision screening. Provides data entry of patients whose case management requires documentation within the program’s internal Latent TB Infection database (LTBI), the Electronic Disease Network (EDN) and the National Electronic Disease Surveillance System (NEDSS).
Monitors patient appointments through the LFCHD’s scheduling system, the LTBI medication logs and the program’s internal LTBI database to assure patient compliance with prescribed treatment.
Schedules appointments in LFCHD scheduling system as necessary.
Communicates with the patients to engage them with medical follow-up via phone calls, to address missed appointments and discuss available resources, such as transportation, and to address any interference with their compliance. Letters are sent and home visits are performed for patients to engage them and discuss resources with those who are out of compliance with treatment and unable to be located otherwise.
Works with PH Nurse Coordinator to prepare case reports for the weekly TB case review meeting. Monitors internet data bases for results from diagnostic and radiology tests, schedules appointments, and assures that all of this information is included in the case reports. The case reports are reviewed by the clinicians and nurses during the case review meetings and a care plan is developed and implemented.
Coordinates follow-up plan of care for children with elevated blood lead levels and perinatal Hepatitis B infections by collaborating with primary care providers to have children with elevated blood lead level evaluated on a 3, 6, 9-month schedule, based on their blood lead level, following the blood lead follow-up guidelines contained in the current KDPH Core Clinical Services Guide.
Public Health Clinic Nursing Duties but not limited to:
Assists with clinical evaluation of patients suspected or diagnosed with TB disease or infection, phlebotomy, apply and read TB skin tests and assist in communicable disease diagnosis and treatment.
Performs a variety of clinic duties such as performing phlebotomy including the collection, packing and shipping of laboratory specimens, reviewing lab results, assisting clinician with procedures, assessing, prioritizing and directing clinic flow, seeking clinical information in person or through phone calls, and administering and dispensing medications.
Provides HIV and Hepatitis C testing, including pre- and post-test counseling.
Provides immunizations, to include assessing patient needs, education regarding immunizations, and administering immunizations following the guidelines and standing orders contained in the current Kentucky Department of Public Health (KYDPH) Core Clinical Service Guidelines.
Serves as back up to clinic Vaccines For Children (VFC) coordinator, including, but not limited to, proper storage and handling of vaccines, assuring adequate clinic inventory, compiling state mandated reports, and participating in site visit audit per DPH guidelines.
Assesses, compiles, and updates immunization data, including Kentucky Immunization Certificates and data in the Kentucky Immunization Registry.
Completes refugee immigration documents.
Interviews guardian, family, and patients to assess needs for determining the nursing process and intervention including contacting outside agencies regarding abuse, and acts as an advocate for the patient and family.
Provides education – preventative and therapeutic – appropriate to educational level, including language barriers, documents in the chart according to the SOAP format, and provides referrals to appropriate resources.
Works in an autonomous role following the guidelines and standing orders contained in the current Kentucky Department of Public Health (KDPH) Core Clinical Services Guidelines.
Education and/or Experience Requirements;
Registered Nurse and Nursing Diploma plus 5 years’ experience; Associate’s degree in Nursing (ADN) plus 3 years’ experience; Bachelor’s degree in Nursing (BSN), or Bachelor’s degree in a related field, plus 2 years nursing experience; or a Master’s degree in Nursing (MSN), or Master’s degree in a related field, plus 1 year nursing experience. Acceptable experience includes: prior health department experience OR experience in 2 of the following areas: Home Health, Clinic/hospital, patients in a confined facility.
Must attend Kentucky Department for Public Health TB Program training within 1 year. Must be willing to attend Southeastern National Tuberculosis Center training if resources allow within 2 years. Current Registered Nursing License. Current CPR.
NURSETBPH: Nursing diploma, Associates, Bachelors, Masters related nursing or related field.
