The Lexington-Fayette County Health Department is an independent agency under the Kentucky Revised Statutes, charged with the responsibility for public health in Lexington-Fayette County. This is a part time less than 23 hours per week LPN position; performs specialized work including various technical nursing procedures within scope of practice relating to students in a school setting and public health population-centered activities under the supervision of a registered nurse (RN).

The pay rate is $18.00 per hour.

ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.
Provides a variety of school health services under the direction of an RN or physician as follows:

o Treatment Coverage Diabetics, etc
o Provides care for sick, injured, and chronically ill students within scope of practice
o KDE mandated vision screenings for K, 1, 3, &.5
o Immunization Consultation
o Assist RN with medication log review and follow-up
o Infinite Campus condition flags placed and updated for students with health conditions
o Assist RN with case management and follow up of students which includes communication with school staff, school health team and families
o Assist with referrals and act as patient/family advocate
o Vision screenings for IEP/ARCs, as time permits
o Communicable Disease Surveillance
o Health Fairs
o Health Classes
o Immunization data entry
Provides patient education/teaching on a variety of preventive health topics.
Ability to help students with diabetes to count carbohydrates and calculate insulin dosage.
Responsible for completing necessary forms for each patient's health records, following program standards.
Assures patient or legal guardian consent for treatment and release of information signatures when indicated, following program standards.
Refers non-routine patient teaching questions to the Public Health Registered Nurse (PHN).
Reports immediately to RN very ill patients and/or unusual situations.
Administers immunization and other injections per physician/clinician orders
Responsible for tracking health records when needed.
Responsible for ongoing stocking of patient education materials.
Responsible for completing statistical reports.
Attends staff meetings and appropriate in-services/training.
Assists occasionally with orientation of new staff.
Participates in community health fairs or public health events.

Graduate of accredited school of practical nursing, licensed as a practical nurse (LPN) in Kentucky and two (2) years LPN experience. Applicable experience includes 1 of the following: prior school health, pediatric cares in hospital or clinic, ER or urgent care, home health, public agency such as Health Department. Must maintain current CPR while holding this position. Requires current and valid drivers license and use of personal transportation to and from job assignments on a daily basis.

Education Requirements (Any)

LPN: Licensed as a practical nurse in Kentucky; current LPN.

Certification Requirements (All)

Valid CPR
Valid LPN license
