Job Skills / Requirements

The Wellspring PAD Team supports adults with serious mental illnesses who are involved with the justice system and have been recently incarcerated or are at imminent risk for being incarcerated. 

The PAD CM participates in regular team meetings and works in partnership with the PAD CM, Peer Specialist, and Program Manager. Working as part of an interdisciplinary team, the PAD CM will assist clients with the coordination of court dates and collaborate with the Office of Probation and Parole and HIP.  The PAD CM also assists clients in gaining access to medical, social, educational, housing, addiction treatment, and other resources ensuring uninterrupted provision of all services. In partnership with the PAD Clinician, the PAD CM develops and facilitates pursuit of individualized recovery goals via assessment, connecting to resources and monitoring progress. The PAD CM completes electronic documentation of services demonstrating medical necessity within 24 hours of client contact.

The PAD CM will have a belief in recovery from mental illness and an enthusiasm for helping persons with disabilities, rooted in empathy and compassion, demonstrating a careful and caring attitude. 

The ideal candidate will have experience working with clients with serious mental illness and substance use disorders. Experience working in or collaborating with the justice system is preferred. 

A personal vehicle is required as the PAD CM will be responsible for transporting clients to regular appointments.

Education Requirements (Any)

*BA/BS in psychology, social work or related field and at least one year post Bachelor's working full-time with adults with SMI
Master's Degree in related field

Certification Requirements (Any)

BA or BS in field recognized by DBHDID for TCM
BA or BS degree with 1 year experience in field
KY TCM Training
